If you would like to request that a fee be waived, please complete the attached form and send it to communitycare@actionlife.com.
Your request will be submitted to the Board of Directors for review at their next meeting. Please keep in mind that meetings may not occur every month, so it may take a while for a response.
Once the Board of Directors has approved or denied your request, a Team Member will reach out to you, inform you of their decision, and apply any credits that may be applicable.
